Abstract
The present study studied the emigration of health workers and healthcare delivery in Nigeria from 2015 to 2024. The main objective was to evaluate how the movement out of Nigerian health workers to destination countries shaped health system of Nigeria, with attention to poor economic welfare and capacity for service. This research used documentary and ex post facto research designs, using secondary data from the Medical as well as Dental Council of Nigeria, the Nursing as well as Midwifery Council of Nigeria, the World Health Organisation, the Nigerian Bureau of Statistics, the Central Bank of Nigeria as well as relevant academic sources. Qualitative content analysis and thematic analysis were used within the push-pull theory of migration. The results found that poor economic welfare was the main structural driver of emigration, as doctor salaries fell in real dollar terms from about $350 to $180 per month between 2015 and 2024, while Medical as well as Dental Council verification certificates reached 4,600 in 2024 alone. This research also found that outward migration reduced the strength of healthcare delivery as skilled health worker density fell from 1.95 to 1.68 per 1,000 population, tertiary nursing vacancies rose to 55%, patient-to-nurse ratios worsened and maternal mortality remained far above comparator countries. This research concluded that Nigeria faces a workforce depletion crisis driven by economic push factors. This point recommended a legislated Health Worker Economic Welfare Protection Framework that links doctor and nurse salaries to exchange-rate movements as well as funded retention measures for healthcare access as well as health system planning.
